The Interdisciplinary Team of Hospice Care

The plan of care for each patient evolves under the direction of the Medical Director who approves admissions to hospice and follows each case. |

Visiting hospice nurses are on-call 24 hours a day. Medical care comes to you in your home, which means no more appointments, emergency room visits or 911 calls. |

Home health aides assist the patient and caregiver with personal care needs for cleanliness, comfort and safety. |
 |
Social workers consult with the patient and family about their practical concerns, fears, future plans and arrangements. |
 |
The Bereavement Coordinator is available to the patient and family for one-on-one support and counseling, and for family meetings. Following the loved one’s death, she provides continuing contact and grief support to the surviving family members and caregivers. |

Our visiting Chaplain is available to help bring comfort and resolution to many of the end-of-life spiritual issues we face. His services can be non-denominational or he can supplement or work in tandem with the care of one’s own clergy. Our Chaplain offers loving, unconditional support and comfort with respect for each individual’s own beliefs and traditions. |

The Family Services Coordinator is available to help with arranging volunteers, equipment and supplies. |

Practical Help

Talbot Hospice provides helpful items and equipment like lift chairs, wheelchairs, hospital beds and more when patients with limited life expectancy are discharged from a hospital or care facility to home health care or hospice care at home. We work with hospital case managers, discharge planners and facility social workers to make sure everything needed for safety and comfort at home is in place. |

Talbot Hospice Foundation’s well-trained and screened volunteers make pre-scheduled visits and provide support to caregivers and patients - offering respite care and gentle helping hands. They can help with light household tasks, transportation, shopping and errand-running, companionship for the patient and peace of mind and understanding for the caregiver. |

How is hospice care paid for?
Medicare and many private insurance plans cover the medical costs associated with Shore Home Care Hospice care. Hospice care and services are available to anyone in Talbot County facing end-of-life issues regardless of age, religious belief, race, insurance coverage or ability to pay.
The supportive programs and services of the Talbot Hospice Foundation are provided without charge through the generous support of our community. Each gift of financial support is important to us because Talbot Hospice Foundation receives no insurance reimbursement from Medicare, Medicaid or private insurance.
To determine eligibility for hospice, services and to enroll in the hospice program, an individual, a member of his or her family, or the individual’s physician should first contact our medical partner, Shore Home Care Hospice at 410-820-6052 x 2900.
